SOCIAL ISOLATION AND YOU

Do you feel tired? Lack motivation to engage in activities you used to enjoy? Feel overwhelmed by all the things on your to-do list? Do you want to feel excited, hopeful, and achieve your goals? Many people feel this way. You might be socially isolated or depressed and besides feeling lonely it can also negatively affect your mental and physical health.

IMPACTS OF SOCIAL ISOLATION

Older adults who feel socially isolated are more likely to report having poor physical and mental health conditions such as depression, chronic lung disease, arthritis, and impaired mobility. Despite these risks, social isolation is often under addressed by health providers.

ACTIVE CONNECTIONS PROGRAM

The Active Connections program helps you to feel better. By working with a trained master level social worker we can help you get active and connected, resulting in more positive thinking and mood.

HOW ACTIVE CONNECTIONS WORKS

Strengthening your problem solving skills, achieving more goals, and engaging in enjoyable and social activities have been shown to improve mood and social connections.

The Active Connections program includes:

  • 8 sessions with a MSW to think through problems affecting your life.
  • The social worker can travel to you or conduct sessions over the phone, making this an easy service to access.
  • It’s free of charge!
